Support Planning

Support planning is the process used to identify the needs and risks of service users who receive support. 

All service users should have individual support plans that address identified needs and risks.

Support plans should be outcome-focused which means they should show what support needs/goals the service user has, how they are going to be met and what specific outcomes have been achieved.

Support plans should be kept up to date and reviewed on a regular basis to show how the needs and risks of the service users may have changed.

Support plans should be put together with the full involvement of the service user and should be signed off by the service user and the support worker. Carers and advocates, where appropriate, should also be involved.

Outcomes and support planning
There is a great emphasis on outcome based support planning. Many local Supporting People administering authorities have introduced outcomes based toolkits and the Communities and Local Government (CLG) introduced the national one this summer (2007).

These toolkits will rely on organisations having good needs assessment and support planning frameworks in place that are able to identify and record outcomes for service users.
